Driving Miss Daisy is committed to providing high quality care and support to meet your needs. We value your feedback – including compliments, recommendations and complaints.

The purpose of this form is to collect information and, if requested, respond to you.

It is important that you provide us with a valid email address if you wish to be able to be contacted or receive any further correspondence about your feedback. You also have the option to remain anonymous, and therefore not receive any further contact from us on this matter.

After you submit your feedback, it will be forwarded to your local Driving Miss Daisy and, unless you have elected to remain anonymous, we will use your email address so that:

You will receive an auto-reply email confirmation.
Within 5 working days, a personalised email will be sent to you acknowledging your compliment, general feedback or complaint and giving more information about what will happen next.
We will aim to provide you an outcome of the investigations within 45 days unless that time is extended to collect information from others.

If your matter is complex it might take longer to provide you with an outcome. If this is the case, an interim response may be provided to you. If you did not provide an email address, we will attempt to contact you using the other feedback information you have given to us.

If you change your contact details please use the Feedback Submission Form to update your contact information for us to keep you informed.
